When you're referred to our Young Carers Service, here's what happens:

  1. Initial Contact: A Young Carer Worker will reach out to you and your family
  2. Needs Assessment: We'll look at:
    • What caring activities you may be doing
    • Whether these activities are appropriate for your age
    • How your caring role is affecting you
    • Your voice and your parent's voice in the process
    • Input from other important adults in your life (if you want)
  3. Support Plan: Based on the assessment, we'll create a personalized plan that might include:
    • Information, advice, and guidance
    • Referrals to other helpful services
    • Access to our support programs
    • Ongoing support from our team
  4. Individual Support: All our assessments, support plans, and services are tailored specifically to you and your family's unique situation
